Организованная частотная структура электрокардиограммы при длительном развитии фибрилляции желудочков сердца в эксперименте

Цель исследования — изучение частотной структуры электрокардиограммы (ЭКГ) методом быстрого преобразования Фурье (БПФ) в диапазоне частот 1–40 Гц при длительном развитии фибрилляции желудочков (ФЖ) сердца собаки в эксперименте.

Материалы и методы. У 10 собак регистрировали ЭКГ до и во время ФЖ. Патологические изменения на ЭКГ до ФЖ не определялись. ФЖ вызывали, стимулируя сердце переменным током. Проводили спектральный анализ ЭКГ при ФЖ методом БПФ в 5 диапазонах частот: очень низкие частоты (1–3 Гц), низкие (4–7 Гц), средние (8–12 Гц), высокие (13–17 Гц) и очень высокие частоты (18–40 Гц). Определяли спектральную мощность (амплитуду) и удельный вес осцилляций (M±m, n=50) во всех 5 частотных диапазонах.

Результаты. ЭКГ при ФЖ характеризуется доминантной частотной структурой. На 1-й минуте ФЖ наблюдаются наибольшие изменения в частотной структуре с переходом от доминирования осцилляций высоких частот (13–17 Гц) к доминированию осцилляций средних (8–12 Гц), а затем и низких частот (4–7 Гц). На 2–10-й минутах доминирование осцилляций низких частот переходит в доминирование осцилляций низких и средних частот. Суммарная амплитуда осцилляций частотой 1–40 Гц существенно не меняется на 1-й минуте ФЖ, статистически значимо снижаясь на 2–10-й минутах.

Заключение. Доминантная частотная структура ЭКГ свидетельствует об организованной электрической активности миокарда при ФЖ. Результаты работы можно использовать в алгоритмах машинной диагностики ФЖ с доминантной частотной структурой. Организованная активность при ФЖ позволяет обосновать поиск способа ритмической дефибрилляции с использованием серии относительно слабых, но ритмованных стимулов.
